    Measles

    An acute infectious viral disease characterized by ‘Koplik’s spots’, high fever, nasal catarrh, cough and blotchy rash.

    CLINICAL FEATURES

    Prodromal stage: Duration: 1-4 days.

    Symptoms of Measles
    Onset:

    • Sudden.
    • High fever, which often starts at about 101 and peaks at 103 to 105 degrees F
    • Hacking cough.
    • Coryza.
    • Sneezing.
    • Photophobia
    • Koplik spots
    • Lachrymation.
    • Photophobia.
    • Hoarseness.
    • Nonpurulent conjunctivitis (pink eye without discharge)
    • Poor appetite.

    Causes of Measles

    Distribution: Worldwide.
    Season: Common during winter.
    Causative organism: Infection with the Measles/rubeola virus.
    Source: Human respiratory discharges.
    Predisposing causes: Poor sanitation, poor nutrition and overcrowding.
    Transmission: By direct contact, droplet infection.
    Incubation period: 8-14 days
    Age: 3-5 years.

    Signs
    Temperature: 40 °C.41 0C.
    Pulse: relative bradycardia.
    Face: congested.
    Eyes: congested.
    On 2nd day:
    – Koplik’s spots become apparent (Koplik’s spots are tiny whitish spots At buccal mucous membrane opposite upper second molar).
    On 3rd day:
    – Temperature: normal.
    Exanthernatous stage
    Duration: 4-7 days.

    Symptoms

    Fever rises again on 4th day.
    Headache.
    Retro-orbital pain.
    Lachrymation.
    Photophobia.
    Hacking cough.
    Rash appear on 4th day.

    Signs:
    Temperature: 40 0C..41
    Pulse: fast.
    Face: puffy.
    Eyes: congested.
    Rash: Pink, maculo-papular. Appears first on forehead and spreads to whole body within few hours. Blanches on pressure.

    Convalescent stage
    Duration: 7 days.

    Symptoms:

    Fever subsides within 1-2 days.
    Rash fades from above downwards.
    Brown stained desquamation.
    Desquamation subsides within 1-2 weeks.

    COMPLICATIONS

    Immediate : Otitis media, Bronchitis, Pneumonia.
    Late : Bronchiectasis, Subacute sclerosing panencephalitis (after 5-7 years).
    (A rare, chronic progressive and fatal neurological complication of measles. It is characterized by insidious onset with intellectual deterioration, apathy and clumsiness, followed by myoclonic jerks, rigidity and de mentia in children and adolescents. EEG. Is characteristic; shows bursts of triphasic slow waves.)

    GENERAL MANAGEMENT

    – Bed rest till fever and rash disappear.
    – Activities like reading, T.V. watching must be restricted till conjunctivitis subsides.
    – Hydrotherapy during high fever, plenty of fluids shoúld be given to the patient
    – Avoid very cold things.

    PROPHYLACTIC MEASURES

    Immediate:
    – Isolation.
    – Children should be sent to school 7 days after defervescence.
    – Prevention of close contacts.
    – Hygienic disposal of infected secretions.
    . Long term
    – Immunisation (as per national health programme).
    – Improve general health.

    Homeopathic Medicine for MEASLES

    Prodromal stage
    Aconite Nap: First stage of measles, sudden high fever. History of exposure to cold air. Child is restless, wants to be carried. Bright red rough rash. Redness of the eyes. Dry, barking, croupy cough, pulse rapid. Very thirsty for cold drinks, Chilly.
    Belladonna: Cold, fever high.
    Euphrasia: Early stage of measles. Eyes are sensitive to light. Constant hot irritating lachrymation. Bland nasal discharge. Measles rash. Pain in the forehead. Worse; sunlight, indoor, warm room. Better open air, blinking, wiping the eyes.
    Ferrum phosphoricum: Fever with congestion of chest.
    Gelsemium: Dizzy, dull, drowsy. Bodyache. Fever. Irritating, watery nasal discharge.

    Exanthematous stage
    Gelsemium: Fever. Measles rash. Irritating, watery nasal discharge. Hard, barking, cough and hoarseness. Headache occipital radiates to forehead. Head feels heavy and hard to lift. Chills up and down the spine. Thirstlessness. Overall weakness. Desire to lie down and go to sleep. Better; lying down with head held high and better by urination.
    Pulsatilla: Later stage of measles. Rash becomes dusky and beginning to fader Child feverish but not thirsty. Desires fresh air, or open window. Changeable emotions. Clingy and weepy. Wants company when sick. Very tearful and miserable. Thick green mucus. Lots of phlegm coughed up in morning. Light hurts eyes. Dry cough at night. Upset stomach or diarrhoea. Bryonia Alba: Rash appears late, or runs an irregular course. The cough is dry and painful. Soreness of the limbs and body, stitches in the chest.

    Antimonium Tartaricum: Child: Irritable, cross, wants to be carried and whines if touched. Cough; much rattling but slight expectoration. Vomiting in any position except lying on right side. Vomiting is followed by drowsiness and prostration. Tongue is coated, thick and white. Desires for apples. Worse; when excited or angry. When eating, and warm drinks.

    Stramonium: When eruption does not come out properly, or disappears suddenly and grave symptoms appear. Child is hot, restless; in sleep cries out as if frightened. Convulsive ovements with red face.

    Zincum: Child too weak to develop the eruption.

    Malignant measles
    Arsenicum Album: Malignant or black or haemorrhagic measles. Sinking of strength. Diarrhoea, delirium, restlessness. Debility. Petechiae. Offensive and exhausting stools.
    Crotalus Horridus: Indicated in black measles.
    Baptisia Tinctoria: Black measles. Foetor and prostration.
    Lachesis: Malignant measles, with individual indications.

    Convalescent stage
    – Lecithin.
    – Kali Phos.
    – Five Phos.

    Prophylactic
    Morbilinum: History of contact with a measles sufferer. If child is not immunized, give this medicine.
    Pulsatilla: It is one of the important drug for immunization.
    Note: If the child’s immune system is already suppressed for some reason. A passive immunization with gammaglobulin, is advised.
    Sticta: An incessant dry and spasmodic cough. Worse; when lying down and at night.
